How to change Sec 80 form in Portal(WDA)

Hi Friends,

I have a scenario, i need to change standard form to my custom form in ESS(EHP7).

1. how do i find out the standard form that is being called?

2. After developing where do i assign it so that when i click on "Print income tax Declaration Request" it should call my custom form instead of standard form.

3. Once employee submits his income tax declaration it goes for approval, so once it is approved then it gets  updated in pa0585 table but before approval if i want to get the data of tax declaration in which table i will be able to fetch from?
